To create a valid healthcare power of attorney in Arizona, an individual must follow certain legal requirements. The document must be in writing and signed by the principal (the person creating the power of attorney). It must also be signed by at least one adult witness or notarized. The witness cannot be the agent named in the document, a healthcare provider, or an employee of a healthcare provider.
